Pre-Service Educator Information Form

The Pre-Service Educator Information Form helps Skokie School District 73.5 learn about a pre-service educator's interest in conducting a field experience, as may be required by their higher learning institution. The Pre-Service Educator Information Form is NOT an application for employment.

This form may be completed by a pre-service educator enrolled at a higher learning institution or by the pre-service educator's advisor or placement coordinator. The District will review the information upon receipt of the completed form and reach out to either the pre-service educator, or the pre-service educator's advisor or placement coordinator, with more information and/or next steps.
