REMINDER!
Skokie School District 73.5 is a clinic site for the COVID Vaccine and Booster. Services are provided by Prism Health Lab at Middleton Elementary School.
Vaccine appointments are available for children ages 5 and older and adults. Booster appointments are available for teens ages 16 and older and adults.
Child and adult vaccines require two doses spaced 3 weeks apart. Adult boosters are a single dose and can be scheduled on any of the four clinic dates. See below for scheduling options.
