2021-2022 School Year Registration

Get more detailed information about the definition of residency, how to prove residency, and to access any affidavits that may be needed to complete the process.

Please complete the 2021-2022 Affidavit of Residency and Verification form.

This is STEP ONE

• Required for all new students when first registering
• Required every year, in the summer, for all returning students

Please be prepared to scan and upload the following:

• Current mortgage statement OR most recent Real Estate Tax bill (if you own)
• Two different utility bills (if you own or rent)
• Current lease (if you rent)

IF you have more than one child attending, you only need to complete the form ONCE. Do not complete a separate form for each child.

e-Registration allows the district to collect and verify all student demographic, emergency, and medical information for your child(ren) prior to the start of the new school year. e-Registration will also allow you to electronically sign the annual permissions, photo and video releases, and device take-home agreements.
 
In mid-July, the district will send an email with e-Registration instructions for your student(s). The mailing will contain a link to the PowerSchool Parent Portal. You will log in to your Parent Portal account and submit an e-Registration form for each student in your family.
 
Families should take action and complete the e-Registration forms as soon as possible after receiving the email. e-Registration must be completed before a student can start school.

Pay fees online or at the Business Office beginning July 14th and AFTER e-Registration is complete.

Below are the Health Forms. Please click on a form to download and for easy viewing. These forms need to be submitted before the start of the school year.

• Emailing the form to the school nurse

• Mailing the form to the school (attention nurse)
• Dropping the form off at the school's front office

ONLY for students that have a need to administer medication during school hours

Certificate of Physical Fitness for Participation in Athletics
ONLY for McCracken students participating in sports including Cheerleading

Permission and Insurance Form for Athletics
ONLY for McCracken students participating in sports including Cheerleading

Child Health Exam
ONLY for PreK, Kindergarten, 6th grade, and all students new to an Illinois school

Dental Examination Form
ONLY for Kindergarten, 2nd grade, and 6th grade

Eye Exam Report
ONLY for Kindergarten and all students new to an Illinois school

Food Allergy, Asthma & Seizure Action Plans
ONLY for students that have an action plan

If transferring from a school within Illinois, students in all grades other than those listed will not need new forms provided up-to-date records are provided from the transferring school.
