Skokie School District 73.5 is partnering with Advocate Children's Hospital and Skokie Health Department to offer a seasonal flu vaccine clinic for students.
The seasonal flu vaccines will be administered by Advocate Children's Hospital staff on board the Ronald McDonald Care Mobile during the school day. Parent/Guardian consent is required for children to participate.
The Ronald McDonald Care Mobile will be at MEYER and McCRACKEN on Wednesday, November 9th and at MIDDLETON on Friday, November 11th.
If you would like to register your child for a seasonal flu vaccine appointment, please complete this Google Form ONCE PER CHILD before 3:00 p.m. on Thursday, October 20th. After submitting the Google Form, you will be able to download a required consent form. Please fill out the consent form and return it to your school nurse by Monday, October 31st. You will be notified by email to confirm your child's registration.
Appointments are limited, but we will work to include all who are registered.
